This session is designed to help care professionals better understand and meet the needs of the person they are caring for. Renowned dementia care expert, Teepa Snow, identifies the key factors that should be taken into consideration when planning a care routine for a person who is living with dementia. Teepa highlights the value of individualizing scheduled routines and programs for the best outcomes.

She places emphasis on knowledge of who the person has been historically—their personality traits, previous life interests and roles, current level of dementia, other medical, sensory, or psychological issues that are present, the environment they live in, and the knowledge and abilities of the helpers. This session also provides structured opportunities for caregivers to help maintain the individual’s skills, abilities, and daily programming as the condition advances and changes are indicated. She also addresses early recognition of changes and effective planning for the person’s future as established habits and routines are lost.

  • Identify key factors to consider when developing an individualized care plan for a person living with dementia
  • Describe how typical progression patterns in dementia may impact care delivery and lead to the need for care plan modifications
